Participants will learn about the Utah Transition Elevated Planning App and supporting materials. This app is designed to help Utah students and family members plan and participate in the transition planning process. As a Utah student with a disability who is 14 years or older, or a family member of a transition age student, this app can help prepare for the Individualized Education Program (IEP) meeting where transition planning will be discussed, and the Postsecondary Transition Plan will be created.
